D2780 Dental Code: Three-Quarter Cast High Noble Crown

D2780 is the CDT code for a three-quarter crown cast in high noble metal, a partial-coverage crown that restores most of the tooth while leaving one wall, usually the facial, in natural tooth structure.

Ask the coverage question before the coding question on this code. Plenty of fee schedules simply exclude three-quarter crowns, or benefit only some alloys of the family, so the same prep can be a covered five-year benefit on one plan and a flat exclusion on the next. Verify the specific plan before the case goes to the lab, because the exclusion cannot be appealed after the seat.

What D2780 covers

D2780 reports a partial-coverage crown cast in high noble metal. The casting restores the occlusal surface and most of the axial walls but deliberately leaves one wall, in most cases the facial, as untouched natural tooth. The fee includes the preparation, impression, lab fabrication, try-in, and cementation, the same visit structure as a full crown.

It does not cover:

  • A full-cast crown covering all surfaces. That is D2790 in high noble metal.
  • A three-quarter crown in predominantly base metal. That is D2781.
  • A three-quarter crown in noble metal below the high noble threshold. That is D2782.
  • A three-quarter crown in porcelain/ceramic. That is D2783.
  • A metallic onlay. That is D2542, D2543, or D2544 by surface count.
  • An all-ceramic full crown. That is D2740.
  • A core buildup under the crown. That is D2950, billed separately.

The material axis, and why this family gets scrambled

The three-quarter family runs on the same alloy ladder as the full-cast crowns, and the two ladders are easy to cross-wire because the numbers do not line up in the same order:

  • High noble (at least 60% noble metal with a 40% gold minimum): D2780 three-quarter, D2790 full cast.
  • Predominantly base metal (under 25% noble): D2781 three-quarter, D2791 full cast.
  • Noble metal (at least 25% noble): D2782 three-quarter, D2792 full cast.
  • Porcelain/ceramic: D2783 three-quarter. There is no titanium three-quarter code; D2794 (titanium) exists only on the full-cast side.

Note the trap: in both families the second code is base metal and the third is noble, which reverses the intuitive cheap-to-expensive ordering. A biller who assumes D2782 is base metal because it sits between the others files the wrong alloy, and the lab invoice contradicts the claim on audit. Read the invoice, then pick the code.

Coverage: the exclusion problem

Three-quarter crowns are an older, technique-sensitive restoration, and plan design reflects that unevenly:

  • Some plans exclude the whole family. The denial reads “not a covered benefit,” the balance handling follows your participating-provider agreement’s non-covered-services terms, and no appeal changes plan design.
  • Some plans benefit part of the family. It is not unusual to see a fee schedule that covers the base-metal D2781 on a per-tooth five-year frequency while listing D2780 and D2782 as not covered, effectively benefiting the restoration only at the cheapest alloy.
  • Some plans pay an alternate benefit. The claim is priced at a full-cast or onlay allowance instead of the submitted fee. Whether the difference can be billed to the patient is a contract question, not a property of the downgrade.

All of this argues for one workflow rule: verify the specific plan’s position on the D2780–D2783 range at treatment planning, before lab costs are committed. A crown exclusion discovered on the EOB is a write-off conversation; the same exclusion discovered at the treatment plan is a patient-choice conversation.

Three-quarter crown versus onlay

The nearest coding boundary is the large metallic onlay, and it matters because the two can price and benefit differently:

  • Onlay (D2542–D2544): a cast restoration that covers cusps and occlusal surface, coded by surface count.
  • Three-quarter crown (D2780–D2783): a crown preparation with axial-wall coverage that spares one face, coded by material.

A reviewer who sees minimal axial extension on the film may reprice a three-quarter crown claim as an onlay. The defense is documentation: a prep description or photo showing the axial walls covered. Conversely, do not upgrade a large onlay to D2780 for the fee; the prep design in the chart has to support the code.

Documentation that supports the claim

  • Date of service at the seat visit, not the prep.
  • Tooth number and a prep description establishing partial-coverage crown design, with the spared wall named.
  • Lab invoice confirming the high noble alloy.
  • A narrative when a crown or major restoration was paid on the same tooth inside the plan’s frequency window, typically five or more years.
  • The clinical reason a partial-coverage design was chosen, for example an intact facial wall worth preserving on a tooth needing cuspal coverage.

Example case

A patient with a heavily restored upper premolar has a fractured lingual cusp, extensive mesial and distal restorations, and a pristine facial surface. The dentist plans a three-quarter cast crown in high noble metal to preserve the facial enamel.

  1. Verify at treatment planning whether the plan benefits D2780 at all, and if not, whether it covers another alloy in the family or pays an alternate benefit. The patient hears the numbers before the prep.
  2. Prep, impress, and note the design: axial coverage on mesial, distal, and lingual, facial wall spared.
  3. Submit at the seat visit with the tooth number and, if the plan requests it, the lab invoice confirming the alloy.
  4. On the EOB, distinguish a plan-design outcome (exclusion or alternate benefit, handled per the contract) from a reclassification to an onlay code, which is worth appealing with the prep documentation.
