The porcelain is layered over the coping for the visible portion. The code applies to permanent teeth only and covers the prep, impression, lab fabrication, try-in, and cementation.
It does not cover:
- A PFM crown with a high noble coping. That’s D2750.
- A PFM crown with a predominantly base-metal coping. That’s D2751.
- An all-ceramic or all-porcelain crown. That’s D2740.
- A full-cast noble crown, all metal with no porcelain. That’s D2792.
- A PFM retainer crown supporting a bridge pontic. That’s D6752 for noble metal.
- A core buildup. That’s D2950.
- A post and core. That’s D2952 (cast) or D2954 (prefab).
- A provisional crown. That’s D2799.
The distinguishing axis: the 40% gold line between noble and high noble
The three PFM codes are separated by one variable, the noble metal percentage of the coping, and D2752 is the middle tier. The boundary that produces miscodes is the one between noble and high noble, because both alloys can contain gold.
- D2750 is high noble: two conditions at once, over 60% noble metal and at least 40% gold.
- D2752 is noble: at least 25% noble metal, without meeting both high noble conditions.
- D2751 is predominantly base metal: under 25% noble metal.
An alloy can clear 25% noble, contain real gold, and still be D2752 because it falls short of the 40% gold minimum, the 60% noble minimum, or both. The presence of gold does not make a crown high noble. Only the two thresholds together do.
When to bill D2752
Bill D2752 when:
- A full-coverage PFM crown is fabricated and seated on a prepared natural tooth.
- The lab slip confirms a noble coping: at least 25% noble metal, below the high noble cutoff.
- The crown is cemented at the delivery visit.
Do not bill D2752 for:
- A high noble crown that meets both the 60% noble and 40% gold thresholds. Use D2750.
- A predominantly base-metal crown. Use D2751.
- A full-cast noble crown with no porcelain. Use D2792.
- A crown serving as a bridge retainer. Use D6752 for a noble PFM retainer.
- A provisional or interim crown. Use D2799.
- Recementing an existing crown. Use D2920.
Top reasons D2752 gets denied or downgraded
- Alternate-benefit downgrade to D2751. The most common outcome on posterior teeth. Many plans pay any PFM crown at the base-metal allowable, treating base-metal PFM as functionally adequate on a molar. This is a benefit limitation, not a dispute over the crown placed. The patient owes the difference, and whether a plan applies it is plan-dependent.
- Miscoded as high noble. The crown is billed D2750 when the alloy met 25% noble but not the 40% gold minimum. When the carrier requests the slip on audit, the claim reprocesses to the D2752 fee and the practice owes back the difference.
- Miscoded as base metal. The reverse error. A noble crown billed D2751 collects the base-metal fee and underpays the office on a more expensive restoration.
- No pre-op radiograph on file. Most carriers require a pre-op film showing the tooth’s condition. Attach it with the initial claim.
- Frequency limit on a replacement crown. Most plans allow one crown per tooth every 5 to 10 years. A replacement inside the window pends without a narrative documenting the clinical failure on the prior crown.
The downgrade and the miscode are two different problems
It helps to separate the two things that go wrong on D2752, because the fixes are different:
- The downgrade is plan design. The alloy on the claim is correct, but the plan only pays the base-metal allowable on that tooth. There is no appeal that overturns a plan-design downgrade. The work is on the patient-side balance and the conversation belongs at treatment planning.
- The miscode is an office error. The alloy class on the claim doesn’t match the lab slip. The fix is upstream, in the workflow that ties the code to the slip before the claim goes out.
A practice that treats every reduced payment on D2752 as a downgrade will miss the cases where the real issue was billing the wrong metal class in the first place.
Documentation that supports the claim
The claim needs:
- A pre-operative radiograph showing the tooth’s condition.
- Date of service matching the seat date, not the prep date.
- The correct tooth number.
- A narrative for anything unusual, such as a replacement crown inside a frequency window.
For the patient record, keep:
- The lab slip with the alloy composition, showing both the noble and gold percentages. On a noble-tier crown this is the document most likely to be requested.
- Prep, impression, and seat dates.
- The clinical reason for the crown.
- Any prior crown on the same tooth and its placement date.
Example case
A 58-year-old patient has tooth #14 with a fractured buccal cusp and a large failing restoration. The dentist preps for a PFM crown and specifies a noble alloy. The lab slip confirms 28% noble metal with gold below the 40% mark, so the coping is noble, not high noble. The plan covers crowns at 50% and downgrades posterior PFM to the base-metal allowable.
Billing steps:
- Verify crown coverage, the frequency status on tooth #14, any waiting period, and the posterior alternate-benefit rule at treatment planning.
- Read the lab slip: 28% noble with sub-40% gold puts the crown in D2752, not D2750.
- Place a core buildup if needed and code D2950 separately on the same claim.
- Submit D2752 at office fee on the seat date, with the pre-op radiograph and a one-line narrative on the fracture.
- Expect the EOB to allow the D2751 base-metal fee and pay 50% of it. Post the payment and bill the patient the downgrade difference and any deductible.
What to get right in your PMS
- Keep D2750, D2751, and D2752 as three distinct line items so the metal class is a deliberate pick. The PFM family is miscoded by one alloy tier more than almost any other restorative group.
- Read both the noble and the gold percentage off the lab slip before coding. The noble-versus-high-noble line needs both numbers, not just one.
- Post on the seat date, not the prep date. That drives the date of service the carrier reads.
- Set the patient estimate to expect the posterior downgrade where the plan pays the base-metal allowable, so the balance is discussed before the seat, not after the EOB.
- Don’t bundle the buildup or post into D2752. Code D2950, D2952, or D2954 separately on the same claim.
